Please understand that payment of your bill is considered part of your treatment. The following is a statement of the Complete Dental Care Financial Policy, which all patients are required to read prior to any dental treatment.
All patients must complete our Initial Visit Forms before seeing the dentist. Please keep in mind that full payment is due at the time of service.
Complete Dental Care accepts the following methods of payment:
- Cash
- Checks
- Visa/MasterCard
Dental Insurance
Complete Dental Care is a Premier Provider with Delta Dental. We may also work with other insurance companies. If we do accept assignment of insurance benefits, we do require that you pay your portion (co-pay or deductible) at the time of your appointment. The balance is your responsibility, whether or not your insurance company pays.
Complete Dental Care is not party to any contract between you and your insurance company. Please be sure to read over and be familiar with your insurance policy. If your insurance company has not paid within 90 days, you will be responsible for the balance in full. Take note that you will be help responsible for the payment of any treatment received but not covered by your policy.
Usual and Customary Rates
Complete Dental Care is committed to providing the best treatment for our patients; we charge according to normal Arvada and Denver rates for our dental services. The client is responsible for payment of any insurance company’s arbitrary determination of usual and customary rates.
Patients without Insurance & Financial Arrangements
Patients without dental insurance are responsible for payment in full when dental treatment is received, unless financial arrangements have been made prior to the appointment. In the event that a short-term financial arrangement is necessary, payment options will be discussed on an individual basis with our Arvada dentist and staff.
Interest Rates & Late Fees
As stated on our Patient Registration Form, Complete Dental Care reserves the right to assess an annual 15% interest charge on all overdue accounts 60 days after the original charge is made. Late fees will be assessed to payments received after the due date on the statement.
